Please download the PDF of your final form, the Medical History Form, complete the entire form and either email to info@drmikeizzo.com or bring it with you when you come for your appointment. If you are unable to download or print the form, no worries! We will provide you with a form to fill out when you arrive - just arrive a few extra minutes before your appointment to do so.

If you have filled out all of the forms, you are done, and we look forward to seeing you at your appointment. Just a reminder that for your safety, someone will be contacting you the day before your appointment to ask you a series of COVID-19 screening questions once more.
